Pinellas County requires every band member to submit the full forms packet to be eligible for the program. The doctor visit (sports physical) is the slow piece — book it now.

How to submit

As required by the Pinellas County School Board, every band member must complete the full set of forms to be eligible to participate in the East Lake band program. All required forms are due by the start of band camp.

Return all forms to Pam Bert (Director's Assistant). Three ways to do it:

  1. In person. Drop them off at the band room during school hours. Pam is there most days.
  2. With your student. Send the packet in with your student on a band class day — they hand it to Pam directly.
  3. Scan and email. If you can't make it in, scan the completed packet and email it to DirectorAssistant@EastLakeBand.com.

Forms feel like a lot, but it's one packet, one envelope, one trip to the doctor. Get it done before camp and the rest of the season is just music.
